About Converging Health
Converging Health was built around a powerful idea: if you can identify rising risk earlier and intervene while that risk is still manageable, you can improve outcomes and prevent avoidable costs.
We work with employers, brokers, and health systems that are responsible for managing population health at scale. Our role is to identify who in a given population is on a rising-risk trajectory, engage them with the kind of consistent, trusted guidance that actually changes behavior, and provide the reporting to show how risk is moving.

Scott J. Thompson
Scott Thompson leads operations at Converging Health, bringing more than 30 years of experience building and scaling population health management companies. He co-founded and served as COO of Genetic Direction and Catapult Health, the latter delivering worksite preventive screenings that catch chronic conditions early. Earlier he ran operations at Viverae (now Personify Health) and WinningHabits (now Optum), two leaders in the field, after senior technology consulting roles at Accenture and Hitachi Consulting. He holds a BBA in Management Information Systems from the University of Oklahoma. At Converging Health, he turns risk intelligence into repeatable operations.

Scott Conard, MD, FAAFP
Scott Conard is a practicing physician, entrepreneur, and executive who has spent more than 30 years at the intersection of clinical care and value-based strategy. He has built programs that prevent disease, engage individuals in their own health, and produce sustainable savings for self-insured employers, risk-bearing entities, medical groups, and health systems. His work organizing physicians and designing care delivery for a 500-plus physician group reflects a career focused on execution as much as ideas. As Chief Innovation Officer, he pairs deep clinical insight with advanced analytics to help organizations intervene before cost and illness take hold.

Mike Tuggy, MD, FAAFP
Michael Tuggy is a rural family physician and Clinical Professor at the University of Washington School of Medicine who has spent his career advancing primary care education and reform. He served seven years on active duty in the US Army, including deployments to Iraq and Kuwait during the first Gulf War, and later was medical director at Confluence Health in Winthrop, Washington. He directed the Swedish Family Medicine Residency in Seattle and, as president of the Association of Family Medicine Residency Directors, helped author family medicine's national strategic plan. As Senior Vice President of Advisory Services at Converging Health, he leads work on payment and practice models that deliver better care at lower cost.

Greg Bellomy
Greg Bellomy is a Senior Advisor with more than 20 years leading healthcare companies as a board member, CEO, and president across behavioral health, home health, clinical services, rehabilitation, and wellness. He has guided growth through new business creation, mergers and acquisitions, joint ventures, and market expansion at both public corporations and private equity and venture-backed organizations. Much of his work has centered on moving payers and managed care toward value-based and risk-based contracting, and on raising the bar for quality, access, and cost-effectiveness. He brings Converging Health a seasoned view of how to build durable, high-growth healthcare businesses.

Larry Bauer, MSW, MEd
Larry Bauer is a Senior Advisor who has spent more than 30 years strengthening primary care in the United States. He led the Family Medicine Education Consortium and was a driving force behind the Primary Care Innovators Network, a collaborative of primary care leaders working to build a healthcare system anchored in strong primary care. Across his career he has been a connector, bringing clinicians and innovators together around ideas that improve how care is delivered. At Converging Health, he lends that network and perspective to the advanced primary care models the company helps organizations adopt.
