We find rising risk while there's still time to change it.

We work with employers, brokers, and health systems that are responsible for managing population health at scale. Our role is to identify who in a given population is on a rising-risk trajectory, engage them with the kind of consistent, trusted guidance that actually changes behavior, and provide the reporting to show how risk is moving.

Meet the Team

The Converging Health team brings together expertise in clinical care, population health analytics, health plan operations, and practice transformation. Our backgrounds span primary care, health system leadership, benefits consulting, and data science. What we share is a conviction that healthcare can be more proactive, more personal, and more accountable than it typically is.

That conviction shows up in how we build our products, how we engage the people we serve, and how we measure the impact of what we're doing.
